Use the brief questionnaire below to see if you may be experiencing some degree of hearing loss. Be sure to evaluate each question carefully and answer honestly to ensure an appropriate evaluation. The questionnaire represents only a portion of the actual evaluation questions HearUSA uses to establish a diagnosis and should not be used to determine a final course of action other than seeking further evaluation by a hearing care professional.

25 to 40 points: Significant hearing loss is indicated. Significant hearing loss results in increased difficulty hearing and understanding in all situations. A complete hearing evaluation is recommended.

10 to 24 points: Mild to moderate hearing loss is indicated. Mild to moderate hearing loss is associated with difficulty understanding speech, particularly in the presence of background noise. A complete hearing evaluation is recommended.

0 to 9 points: Little or no hearing loss is indicated. However, a professional evaluation can determine your hearing baseline.
